Our care team works collaboratively with members, healthcare providers, managed care plans, and community organizations to identify barriers and create individualized plans of care.
NOVA delivers the full set of ECM core services, documents care plans and outreach, and coordinates directly with plan care management staff on member assignment, engagement, and transitions of care.
